Johnson was born in Greenwich, London, on 23 August 1984. His father, David, was a professional footballer who played for West Ham United, Millwall and Southend United. His mother, Susan, was a schoolteacher. He has two older sisters, Gemma and Rebecca. Johnson was educated at Plumstead Manor School and Woolwich Polytechnic School.
Johnson began his career as a trainee at West Ham United in 2001. He made his first-team debut in 2002, and went on to make 33 appearances for the club. He spent time on loan at Millwall in 2003, and made 11 appearances for the club. He also spent time on loan at Chesterfield in 2004, making four appearances for the club.
Johnson moved to Chelsea for £6 million in 2003. He made his Chelsea debut in a 2-1 victory over Manchester United in August 2003. He went on to make 97 appearances for the club, scoring four goals. He won the Premier League title with Chelsea in 2005, and the League Cup in 2007.
Johnson moved to Liverpool for £17.5 million in 2009. He made his Liverpool debut in a 2-1 victory over Tottenham Hotspur in August 2009. He went on to make 200 appearances for the club, scoring seven goals. He won the League Cup with Liverpool in 2012, and the UEFA Champions League in 2019.

Johnson moved to Stoke City on a free transfer in 2015. He made his Stoke debut in a 1-0 victory over Norwich City in August 2015. He went on to make 85 appearances for the club, before retiring in 2019.
Johnson was capped 54 times by England between 2003 and 2014. He was part of the England squad that reached the quarter-finals of the 2006 FIFA World Cup.
